Win XP will only format completely if after u boot and hit f8 or del (varies on brands) at initial boot b/4 windows start, it will ask you if u want to repair win and leave personal files or reformat deleting everything meaning u must have another OP disk nearby or in drive.
The dos commamd format c:/ can't work unless u have op sys on another drive besides c:
 
Once you format w/98 disk u must set partitions,if neccessary, del any extended partitions, set active partitions and name set partion size(if under 138MB).

I know I've done it before. I just could not recall the correct command.
I got a reply with the correct command. If Win 98, you boot from the Win disk then type from A:\ format c:, but I guess Win XP will format during the install.
