Consider, 2% zilocaine in a tube.  A little goes along way.  A little on the 
tip of the cath before insert will aid in the process.  You could be having a 
spasm of the muscle
that controls the bladder.  A couple of very slow deep breathes will help 
relax the muscle during the cath process.
