I know we can get free long-distance by a doctor signing a paper to the phone company.  This might be helpful although I have not read yet.  We're not always at our home phone.  And I think there is in the verification you have to provide through Medicaid card.  Can't exactly remember what I had to provide.  Maybe I'm getting confused with the lifeline feature of a lower phone rate.  I know I have to do every year for that feature.  I think the physician signing is for free 411 on your home phone.

In Illinois and Indiana, that service is free, with subscription, for 411 service only.
It doesn't travel with you from one phone number... to another.  I believe that you
would have to go thru the qualifications for each phone number you use, before
you actually have to use it.
