i'm using both... straps to start and get them high enough for hands to reach, then spasms can take over!

Back when I was doing transfers, I had my wife sew a strip of the same kind of material on the seam of my pant leg below the knee.  It didn't show, and I could hook my thumb into it to pull my leg up.
