You may have answered your own question.  Sounds like your insurance  company 
may be sitting on the idea and that is truly a shame.  We were much  luckier. 
 Our Neuro did his residency at Northwestern years before he came  out to the 
surburbs and when he placed his call, it was directly to Dr. Betts,  the 
Executive
Director at the Hospital.  Talk about greasing a pathway, lol.   Dr. Chen was 
still a resident of the 3rd floor seeing patients daily.   Today, Dr. Chen, 
head up the Spinal Cord Injury Department at RIC.

Yes, I did have a referral from my primary care physician,  and I think the 
whole problem and hold up is with my insurance paying for it.
