Have you tried contacting your local land-line customer service department  
for their assistance. Tell them that you are looking for a hands-free 
device  with duplex vs simplex capabilities. If you know of or have a Rehab OT 
in 
your  area, that is part of their training.  Much depends on a person's 
abilities  and capabilities.
