I have yet to meet an old electric wheelchair that could not be repaired or 
 restored.  My advise it to contact Medicare/Medicaid  (CMS) in your  state 
for the updated procedure of obtaining a new chair.
 
Yes, it begins with a face to face with your doctor.  Hopefully a MD  that 
specializes in Rehab Medicine.
Yes, a face to face with a licensed Occupational Therapist.
Yes, a doctors order for new chair.
Depending on your physical condition, your medical needs and finding a  
manufacturer that can meet those goals, it can take several months.
