In my opinion I would suggest:
Starting with the Caregivers, the thigh straps on your sling should be kept
closer to the knee then to the groin. For me, the thigh straps go through
each other and anchor to your Hoyer on the opposite side of the leg that it
supporting (I.e. the sling that goes under the right thigh goes through the
sling that goes under the left thigh, and the right sling connects to the
left hook and the left sling connects to the right hook. When you keep the
sling low towards towards the knee and thread them through each other it
should keep the sling away from the scrotum, otherwise the sling can
pinch/rub/bruise the scrotum. You should watch for this when they're
putting you in your wheelchair and especially your shower chair.
After I'm in my wheelchair and been positioned correctly I have my
caregiver checked my thigh strap that holds my catheter to make sure it's
towards the knee otherwise if it's too high the scrotum can rub on it and
cause irritation. I have them check the catheter so that it's not kinked or
crowding the scrotum then have them lift my "junk".

I like sitting in the sun in the summer and winter and I find that if I get
to warm the scrotum tends to give me problems. Anything from being red, raw
like a slight rug burn, or getting a few very small blisters but I have
never had a sore/wound. So I try to stay out of the sun or getting to warm
for a while. If it's not gone overnight or in a day or 2 I have let it air
dry during the day, by going without pants and with a sheet or towel over
my waist.
I would think that doing few extra weight shifts may help also, but maybe
use recline instead of a tilt position.
Don't be afraid to let you caregivers know what you would like them to do
and why (nicely) and be open to suggestions should they have any.

It seems like we're always having problems of some sort and it's easy to
let things slide but probably only gets worse.
