If it was done fast (Off then immediately On again) that is the test 
one does to see if it's a dead PRAM battery.  What I think happens is 
that when you turn it on the first time it clears the PRAM but not 
until after it attempts to read it to boot (and therefore fails to 
boot).  When you turn it off and then on again the power to the PRAM 
is retained by capacitors and the system attempts to boot and sees 
good PRAM data so it does boot.  Depending on the machine you can 
also do this by restarting the computer after turning it on.
