New issue 1599 by epmclellan: Latitude and longitude fields visible only
when editing ISDIAH contact, not when creating it

To reproduce this error:

1)Create a contact in ISDIAH
2)Save the record
3)Edit the contact
Resulting error:
================
Latitude and longitude are visible only on edit
Expected result:
================
Latitude and longitude should be visible when creating a contact, or should
not be visible when editing contact. ISDIAH does not require latitude and
longitude (see ISDIAH 5.2) so it is not necessary to include these fields.
