New issue 2083 by [email protected]: Enter donor name into donor
dialog box and contact information and submit. Donor name appears in
accession record. Open donor dialog to edit information and Donor name is
missing but contact information is there.

To reproduce this error:

1)create accession record and open donor dialog to add donor name and
contact information
2)submit
3)edit donor dialog
Resulting error:
================
donor name is missing from dialog but contact information is there
Expected result:
================
donor name should appear in dialog
