notes" row (RAD template)

Currently, to enter more than one note in the "other notes" area (general
note, physical description note, etc.), the user needs to save the record
and then go back to the editing form.
While the way RAD notes are handled has been identified as something
needing to be addressed more generally, adding an "add new" link (as for
add new child levels, for example) would be a useful enhancement.
