I've been using Rimu Hosting (rimuhosting.com) for a couple years or more and I love their service. They are hands down the best web host company I've ever dealt with.

With the new reserved instance prices at Amazon, their EC2 service is looking great if you want to pay for a year or 3 year contract. After my experience with them at work I'm leaning more that way every day. It's nice to setup a rails environment and then just replicate it rather than starting with a clean Cent OS server.

Considering email, I also agree that Google Domain Apps is the way to go. It's free if you only need about 25 accounts or less and don't mind ads. You still get the same TLS support for pop, smtp and imap. Managing a mail server is a pain so I wouldn't waste my time with anything else.

I highly recommend Linode VPS. Granted, its not for everyone, one has to wear his sysadmin hat for that. But it does provide you with freedom to pick what services you're going to run on that box, like your own mail server. Static IP included. Quite affordable.

I've heard good things about Media Temple.

For shared hosting, my experience is limited to Site5. We had some issues related to fast-cgi setup (doesn't everyone?). Lately, they switched to mod_rails, so I expect it will get better. Their support is quite responsive.
