[Radiant] Best-place to monkey patch Radiant extensions?

If I want to monkey patch a Radiant extension, where is the best place 
to do it, e.g. a place where I can be sure that all of the extension 
have been loaded.

If anyone can point me to a reference on how load order works in 
Radiant, that would help a great deal.
