Thanks for sending the configuration file and logs, it makes it *much* easier.
The problem you have is due to the Accounting packets not being processed by
the AuthBy DYNADDRESS clause, and hence the accounting stops aren't causing the
IP addresses to be released.
I must confess I don't understand what you are trying to do with your
configuration file, but I suggest you simplify it by defining your AuthBy's
first with Identifiers and then using the Identifiers in your Realm.

Identifier CheckNumber
.......
</AuthBy>
<AuthBy SQL>
Identifier CheckUser
......
</AuthBy>
<Realm DEFAULT>
AuthByPolicy .....
AuthBy CheckNumber
AuthBy CheckUser

</Realm>
