I don't have any ideas, but am curious. Your LMS UI (or your PC) should mimic what is happening on your radio (with your radio selected as the player). So, I am wondering, if you try as you say to play an item from your playlist via the radio UI controls, what happens on your PC LMS UI? Does it give you more than a blank box? I ask because I have an unrelated, somewhat similar thing I am trying to debug.
There is also the LMS server log. Server Settings-> information -> bottom of the page Server Log File. See if it records an event related to the box. I would also make sure you are running a recent version of LMS (I run 8.2, many here run 8.3), and also make sure your radio firmware is up to date. Also (don't know if this matters) make sure you have the version fix patch. If you are unsure of this, let us know we can point you.
