Well the data model doesn't have any concept of replying to a comment so I'm
not sure how we would implement this - presumably you're thinking of any
comment that doesn't have a later comment from the changeset author?
I suspect that might be quite an expensive query to do and I also wonder how
much noise there will (false positives if you like) from comments that don't
need a reply - all discussion has to end somewhere after all.
