I can highly recommend using SecurePay as a direct credit card billing gateway. Their support staff are sharp, and integrating with them using ActiveMerchant in a Rails app is a breeze. The only hurdle is getting setup with a merchant bank account with Internet facilities enabled, but you'd have to go through that for any payment gateway (besides PayPal). At least if you're using Commonwealth Bank... it was a huge pain in the arse for us.
We launched http://europeanwine.com.au (shameless plug) with PayPal as our only payment option while we jumped through hoops getting merchant banking setup, but it wasn't very popular. As soon as we got direct credit card billing going with SecurePay our sales took off! It's a shame really, PayPal was very simple to setup, but it's expensive and adds extra friction to the checkout process.
