We've experienced this kind of problem from time-to-time running on Windows 2000, sometimes on terminal servers with Citrix Metaframe. Recently one terminal server at a given location would not run any programs from the menu - always hit the "instruction could not be read" error. The other terminal server at the same location worked fine.
I got to poking around and discovered that the PATH had grown to be many lines long. Through the installation and removal and reinstallation of other software several directories were referenced multiple times. I cleaned up the PATH setting, thereby reducing the size of the environment, and the R:Base problem cleared up.
I stayed 4 hours with client, and during that time, RBW made severals access violations and return to Windows. Each time, I got Windows' message 'RBG65.EXE has generated errors et will be closed by Windows. You must start again the program. Error log journal is being created'
