Hi Larry and Alastair,
The program is installed into a subdirectory of c:\program files, created by the installer.  A shortcut is also created and points to the .exe from the rbase compiler, so that is all as it should be.  I didn't intentionally use any user directories, and do not see any evidence of a faulty installation...unless the vista hierarchy uses c:\program files as a user-owned directory...but that doesn't seem logical or likely.  I did confirm the following:
 
1.  All program files are in the locations they are supposed to be.
     a.  I did completely delete the old files.
     b.  I ran the installer and the newly installed files were in the expected locations.
     c.  A search revealed no additional .rb? files anywhere else on the disk.
2.  The problem is duplicatable...the program works perfectly when launched "as an administrator", and shows the old form when it is run as an ordinary user...even though that form no longer exists in the database!
 
The odd effect happens even if the .exe is launched by double-clicking it directly and not using the shortcut at all. 
 
It is my belief that this is a vista issue (some kind of "helpful intervention" that is so common in vista), as my RBase compiled program works flawlessly on XP.  I hope we can identify precisely what the issue is...could shed a lot of light on future "issues" with vista!

Have you used any of the "user" directories in your installations?
 If so, they can look the same but they're not.

Also, the Desktop is in the USER hierarchy.  If the application is installed to the desktop or (AND THIS HAPPENS SURPRISING OFTEN) if someone dragged the actual EXE file to the desktop instead of a shortcut, you might see this happen.
