I went back and found my Bug report at RDCC. In my case, it was in a move from 9.0 (64) to 9.1 (64)
In my case, going from 9.0 to 9.1, the broken forms were corrected by an unload using 9.0, connection in 9.1, and running the unloaded file in 9.1.
