Dayle, Scan the pre-printed form to create a jpg or bmp of it.
Pull down the Reports menu and toggle "Page Style" on You will find a new "Page Style" band in the report design. Think of this as a background or watermark for your report. Make it the size of a sheet of paper, and place your scanned image of the pre-printed form on it. Then spend a while with some trial and error making everything line up just right.
