I found the culprit as to why my form dates were not showing properly.
My time settings (Sequence) were set to HHMM.
When I changed that setting to HHMMSS, presto!!
My form dates now show properly.
So often it is something simple.

I unchecked both of the boxes you showed below but the dates all still
change when I get out of the DB then get back in.
Is there another setting in the CFG file that maybe overrides these
settings?
This just started at the beginning of the week.

The only thing that changed was I installed the latest update over the
weekend.

Here's how:

01. Start R:BASE eXtreme 9.5

02. Database Explorer | Main Menu | Settings | Form Designer ...

     Un-Check the option for Autosave When Run
     [ ] Autosave When Run

     Click on [OK] Button to Save Settings

03. Database Explorer | Main Menu | Settings | Report/Label Designer |
     Default Settings ... | General

     Un-Check the option for Autosave When Preview
     [ ] Autosave When Preview

     Click on [OK] Button to Save Settings

That's all there is to it!
