I don;t know if it is the APPROVED way, but I have a cludge that works <g>
Connect to the old database
Output filename
Unload the data SYS_Forms or Sys_Forms2 table as is appropriate
R: unload data for sys_forms2 where SYS_FORM_Name = 'FormName'
output screen
connect to the new database
load SYS_Forms or SYS_Forms2 from Filename
Then do the same thing for reports
Report tables are SYS_Reports and SYS_Reports2
DON'T try to mix the sys_forms and the sys_forms2
The 2 at the end of the name signifies the Windows version. 
If you're copying to another directory, be sure to take the .LOB file with 
it, or it won't work in windows. I'm not sure if the lob file is strictly for 
forms, and reports uses another extension or not.
However, before I did ANYTHING, I would make a backup so you can play and be 
sure it works OK.
Not that I would EVER louse up my databases by playing, and need to restore 
that copy <g>
