The headers are just look up values from my look up
table s = ^^^Subjective^^^, H = ^^^History^^^^,
PH = ^^^Past History^^^
 
The ^^^ are the carriage returns to make blank lines
after the headings.
 
My form looks like this
Custnum
Date
Code
Note field
 
In Code I type in ph and press enter, there is a lookup table
where ph pulls up ^^^Past History^^^, this canned text is
inserted into the Note field.  A EEP takes me back to
the Code field, then I type in S1, that looks up the canned
text... The patient stated they do not have a past hist of....
 
If I type something in the code field that does not have a
lookup that text is inserted to the end of the text in the
Note field.

Marc, Thanks for the detailed response.   My original thoughts were to have one big varchar because you can edit it with some formatting in RTF.  But my problem became how to insert those headers.  How do you do this in your form?

In my 6.5 app I use 1 note field and use a lookup table to
insert canned things like  ^^^Subjective^^^, ^^^Past History^^^
(the ^^^ is a carriage return)
 
On my form I can type in a code like PH in a field
and ^^^Past History^^^ is inserted into the note field.
Then I can enter something like S1 which will insert
something like ...[Fname] came into today for......
( I do a search and replace for [Fname] )
Then you can type in what ever you want and the
text is added to the end of the note field, of course
you can click into the note field and edit the text too.
 
I am going to switch to a varchar in 7.1 so I can have
more than 4k for the notes.
 
I do not know if having a separate field for each
category like Past Hist, Current problem.....
will be helpful?
 
I think you can use those dictating programs inside
a note field and let it type / transcribe for you.
I did a small test a few yrs ago and it worked OK.
 
Let me know if you come up with any good suggestions.
So far I am going to go with 1 big varchar to store everything.

Hi all,

I am back to creating a medical dictation system that has many categories for a single dictation.

For example, some of the fields would be "Current Problem", "Past History", "Physical Exam", "Plan", etc, etc.


All of these fields have a variable amount of data, from a sentence to paragraphs.  I would like to use a combination of note  and varchar fields in a table for these fields, maybe 8 or 10.  Is this going to be a good thing to do???


I heve never put more than one note field in a table, and in the past have tried to isolate the field into a different table in case of pointer corruption, but in this case.....is this a safe thing to do??  Lately I have not had any corruption issues.  Is there a better way to do this?  Opinions all welcome!
