I got this message when I tried to load a very vanilla FORM while in Forms Designer. None of my other 30 forms seems to have a problem.
I run autochk before I make my daily backup and it passed muster but my backup now has the same problem.

Using the Database Explorer, UNLOAD the form and then send me both .FRM and .LOB files.
. Database Explorer | Forms | Highlight that form . Right-Click | Unload ... . Keep the default formname.frm . Click on [Save] button
E-mail me privately both .frm and .lob files.
