Just a datapoint:

When I received my Rivendell Road in mid-1999 (just in time for 
Paris-Brest-Paris), it was one of the first with the "new" (Grant 
P.-designed lugs) and one of the earlier bikes with custom features. 
In fact, I was surprised by the change in lug style, as I rather 
liked the first generation lugs.

I recall that when I placed my order while visiting Grant's family in 
late 1997, custom features officially were not part of the picture, 
but Grant was going to make an exception for my bike. So basically, 
the early lugsets (Richard Sachs-designed) roughly coincide with the 
"standard" models, where you could choose size, short vs. normal top 
tube length and a few extras like a third bottle cage and low-rider 
bosses, plus the color from the standard Rivendell palette. My wife 
has one of those early bikes, Waterford-built.

Mine was a Joe Starck-built machine.

The Heron became available when the Rivendell went custom and more 
expensive, around that time. The Heron then became the 
Waterford-built production frame, with even fewer options than the 
early Rivendells. After Heron was sold to Todd Kuzma, Rivendell 
introduced the Rambouillet and Atlantis to take the place of the 
Herons. There was a hiatus of 1-2 years when Rivendell didn't sell 
production/standard models.

So my recollection is:

1994 Rivendell Standard introduced as one of a few models
1997 (?) Rivendell LongLow introduced for standard-reach brakes. I 
remember riding the prototype at Grant's
1999 (?) Rivendells went custom, Heron introduced around then.
200x Heron sold to Todd Kuzma
200x Rambouillet introduced.
