Have flown three times with a bike. Never was charged extra. Then again, it's a Brompton folder in the padded bag. TSA usually inspects the bag. Never been penalized for hauling a bike without declaring it before. Last time, though, TSA didn't put the pedals back in the side pocket and they bounced around the bag for the whole trip. Luckily no damage done.
However, it's not the most elegant bike solution. Have to admit am jealous of bikes with couplers. But just don't fly enough for it to be worth the expense. (Until two years ago, hadn't been in a plane in 25 years.)
