So sorry about your problems, Liesl, but all miles are good miles - even those 
upright ones!

As for the stem issue, this combo - quill adapter, and threadless stem piece -- 
from VO  is, IMO, quite attractive - though more expensive than the Nashbar:

 .  I think there are similar less expensive combos.

Shoji and Jeremy are correct, the V brake lever won't work with cantilevers 
unless it has the adapter built in.  Unfortunately, the set of Soma Oxfords I 
have sitting around have Tektro V brake levers or I'd off them to you to try.  
Let me know if the other swap is too much of a hassle.  Am pretty sure I have 
some cantilever brake levers sitting around and would be happy to let you use 
them and the handlebar to try things out for a while.

And like others, sorry to hear about your current issues.  I've got some low 
back pain trouble now, so I can be extra sympathetic.
