I can not check "important" for more than one item on the survey

Something I'd always thought should be gathered is statistics on who
started using RB when, what version they last bought, and what's
keeping them from upgrading to the latest version if they have not
upgraded for a while.
So, instead of just saying someone should do this, I paid for a
subscription to a survey hosting service and will run my own.
It might not be a statistically valid sample, but it may at least
coalesce some of disparate feelings into one set of numbers that I can
send to Geoff and Matt.
