I just posted Classic App Fixer, which automates the procedure for
fixing these and other issues.
I encourage anyone who builds Classic Mac applications with RB2005 or
later to check it out at:

This works around several issues that are not solved by including a
"Resources" file in the project, and it's pretty easy to set up. Once
you set it up, just drag a built app to Classic App Fixer to repair
its resources and file flags.

And no, it does not appear that you can have RB do this automatically
correct. (It used to do this, but RB 5 or 200x broke this)

I thought I was on the right track to solve this but now I
discovered that RB 5.5 built apps DO NOT have the BNDL bit set and
they DO have UseCustomIcon set, which is exactly how Rb2006 sets
them. I checked several of my RB5.5 apps, where the icons seem to
work and the BNDL and Icon flags are set like RB2006 sets them.
One difference is the RB5.5 app also has the INITED flag set
whereas it's not set in the Rb2006 built app.
I wonder if RB5.5 also was setting them incorrectly, and the
breakage occurred somewhere before Rb5.5?
