I would guess the latter in most cases. They're not so hard to program, but coming up with one style that pleases a wide variety of customers isn't easy. Take a look at the screen shots you posted; they wouldn't look good unless they were part of a larger set of controls that all shared similar backgrounds. And most developers don't want to deal with highly stylized looks like that; it always ends up being a lot of trouble. For one thing, those realistic renderings (some of them look like photographs in fact) would require a lot of pre-rendered pictures, which complicates their re-use in RB projects. That said, the standard RB slider does suck a** on Windows, and it would be nice to have some common variations on the generic slider, like tick marks and detents.
