Q1. Is it generally hard to implement a folder structure layout that mimics the navigation structure and does either version 9 or 10 introduce anything new that makes life simpler in this area. Is the issues more with LiveServer than with CMS?
Yes, its a messy thing to do, but it can be done, versions 9/10 do not introduce anything which can help you. The issue is within the CMS system, it does what it sets out to do but due to the way reddot publishes files it causes problesm, for example, the parent page of any children page (for example a hub page) will get published OUTSIDE the expected folder by default so if you want a folder structure where you can just goto the directory (eg mysite.com/news ) you will need to add the hierarchy into the filename for those pages as you mention later in your post. This in itself means that you have to create the folder structure before you publish the pages as reddot wont create any folder structure if its in the filename if one doesnt exist. Q2.
