Hey Daniel, This is exactly the problem faced by Tourism Australia - www.australia.com. We have 17 Language Variants and Content Translation Manager plugin deals with sending translations to a 3rd party agency. We built an automated translation system that uses WorldLingo (but could use any trans API) for an Italian company years ago called Wayin. It's OK for getting a sense of how big your content will be but it's not production ready. That could also plug into a translation provider (human) if need be.
We manage an XML with the language/region combinations and have ASP.NET manage the switching. Each language gets its own sub-folder (except Eng). Akamai does the country detection for us for setting defaults, but you could (should) use the browser supplied language instead. Few thoughts...
