I've built themes for both (about 10 for Joomla and 3 for Drupal.)  I am 
partial to Drupal in general but I've found Joomla themes easier to 
build and more flexible.  My process is to use a photoshop document 
created by the designer (or create one if I'm designing, which I try not 
to do because I'm not very good at it) and use that to build the XHTML 
structure and the style definitions for everything that appears on the 
site (and common tags).  With Joomla (more than Drupal, I think) it is 
not-terribly-difficult to modify a theme that has the structure you want 
by changing it's CSS and images.  That is definitely the way to go the 
first time or two.  There is is some PHP involved (and a line or two 
added to the XML if you change the structure) but the cut-and-paste kind.
