It was quite easy to install using the FBML. Mailchimp has documentation for installing the script: http://www.mailchimp.com/blog/subscribe-form-facebook/ If you're using a form other than an email subscription I use Wufoo. Wufoo is a very easy to use form creator that produces just about any kind of embed code you can imagine. The reports and notifications from Wufoo are also great - I particularly like the text message notification option.
