I don't know what band Nick is trying to operate on, but my suggestion would be to get a purpose-built vehicular repeater, like a Motorola PAC-RT, and operate that way. Maybe Uniden has something similar - I'm only familiar with Motorola...
FWIW, Nick, the PAC-RT operates on a completely different band than the primary radio (for example, the primary is VHF and the PAC-RT is UHF), so you would need two antennas, but no duplexer. So your UHF portable would transmit to the car, which in turn would transmit on VHF.
