New issue 2175 by sanderberents: review comment checkbox and table of
contents

It would be helpful if each review comment could be 'checked off' or
otherwise be tagged or marked as accepted/rejected to ensure all issues are
properly addressed. In addition, a kind of table of contents should be
generated between the review request box and the first review comment box,
which would display statistics like 'X out of Y review comments marked', in
addition to quick links to the review boxes (if there are more than one).
