[SPARK-5817] [SQL] Fix bug of udtf with column names
Column names doesn't set properly for UDTF, the following case will throws
exception
```
createQueryTest("insert table with generator with column name",
"""
CREATE TABLE gen_tmp (key Int);
INSERT OVERWRITE TABLE gen_tmp
SELECT explode(array(1,2,3)) AS val FROM src LIMIT 3;
""".stripMargin)
```
