[SPARK-11654][SQL][follow-up] fix some mistakes and clean up
* rename `AppendColumn` to `AppendColumns` to be consistent with the
physical plan name.
* clean up stale comments.
* always pass in resolved encoder to `TypedColumn.withInputType`
