We have a pair of web servers running RHEL5, and httpd crashes on them
about once a week (not on a regular schedule) with the following
errors in the error log:

[Wed Jun 27 08:02:06 2007] [notice] suEXEC mechanism enabled (wrapper:
/usr/sbin/suexec)
[Wed Jun 27 08:02:06 2007] [error] (28)No space left on device: Cannot create
SSLMutex
Configuration Failed

The immediate solution is simple enough, and can be found at
http://carlosrivero.com/fix-apache---no-space-left-on-device-couldnt-create-accept-lock
for those curious. However, rather than setting up a cron job to
remove the semaphores and restart apache every night (ick!), I'd like
to figure out what the root of the problem is so I can prevent it from
happening in the first place.

Anyone have any ideas where I could start to look? Thanks!
