The solution was to add
AcceptMutex flock
to my Apache config. Hope that helps someone else in the same
situation.
We've seen this on two RHEL5 machines now, so I suspect other folks
will see it as well.

The immediate solution is simple enough, and can be found at

for those curious. However, rather than setting up a cron job to
remove the semaphores and restart apache every night (ick!), I'd like
to figure out what the root of the problem is so I can prevent it from
happening in the first place.
Anyone have any ideas where I could start to look? Thanks!
