I have my management scripts do a "yum clean all" before loading some
things to make sure the caches are clear. On all my RHEL 5 systems with
additional repos (EPEL and a local repo), I get:

# yum clean all
Loading "rhnplugin" plugin
Loading "security" plugin

Repository epel is listed more than once in the configuration

It isn't listed twice anywhere that I can find, and if I manually clear
it out (i.e. rm -rf /var/cache/yum/epel), it works. If I have two
additional repos, I have to manually clean both out or I'll get an error
for one first, clear it out, and then get an error for the second.

Any ideas? Anybody else seen anything like this?

It's a bug in the yum rhn plugin. It didn't used to clean anything, now it only works if you don't add any additional repos. I've got an open support ticket on it right now.
