I've never had this problem with files that were encoded directly to flac but I had exactly the same problems with flac files that had been converted from apple lossless files (using dbpoweramp).
Your screenshots show flac files in the itunes directory - did you convert from apple lossless to flac? I found that whenever I used mediacentre to change a (m4a to flac converted) song's rating, that album would then be broken up in Slimserver in exactly the same way you are describing. I removed a lot of the extraneous apple tag info from the flac files and Slimserver is happy and everything works fine.
