Both your issues can be resolved in the EAC - EAC Options - Filename
menu.
In the Naming scheme field, enter in:
Code:
--------------------
%A\%C\%N - %T
--------------------
which would place the file in an Artist\Album folder and name it "Track
number - title".
You could also use variants like:
Code:
--------------------
%A\%C\(%N) %T
--------------------
which would place the file in an Artist\Album folder and name it
"(Track number) title".
