I've had exactly the same problem with an external BTC DVD Drive and iTunes, this was on Windows Server 2003...
I'm not sure what the problem is but it probably is an Itunes + driver issues. My problem was resolved by using another CD / DVD drive =:-0 It may possibly be resolved by using something different from itunes.... I'm now using EAC where I have never seen any of these issues. You cannot rip to Apple lossless directly from EAC (I Think).
