First, you need to set the server option to treat multi-disc sets as a
single album.  By default this option is turned off.

Is "(Disc 1)" actually a part of your album name (in the ALBUM tag)? 
SqueezeCenter uses the album name to differentiate albums, so that
won't work.  The names need to be identical if the discs are to be
grouped as a single album.

Directory structure is also very important.  SqueezeCenter uses folder
paths to help tell albums apart.  The tracks from all discs in a single
album need to be in the same folder.  I've had luck using subfolders for
individual discs in multi-disc albums, so there's a bit of little
leeway.

- Use either DISC or DISCNUMBER tags to add the disc numbers, or it
will all appear as one single gigantic disc.

- If ARTIST tags differ, then you'll need to have explicit
COMPILATION=0 tags to keep the album from being seen as a compilation
and being grouped with other Various Artists albums.
