I wouldn't worry too much about the inefficiencies of option (2). If you look at the relative sizes, adding a 10-50KB JPEG to each 20-50MB FLAC isn't that horribly inefficient.
And certainly option (1) *is* compatible with some apps. Foobar2000 and MusicIP work just fine with cover.jpg -- too bad MediaMonkey doesn't support it. I would venture that a lot of other apps don't support option (3), so before going down that route you want to be sure that you'll be staying with MM.
