I wouldn't worry too much about the inefficiencies of option (2). If you
look at the relative sizes, adding a 10-50KB JPEG to each 20-50MB FLAC
isn't that horribly inefficient. 

And certainly option (1) *is* compatible with some apps. Foobar2000 and
MusicIP work just fine with cover.jpg -- too bad MediaMonkey doesn't
support it. I would venture that a lot of other apps don't support
option (3), so before going down that route you want to be sure that
you'll be staying with MM. I've never tried the link approach, so I
can't help you on how to make that one work.
