I've had to do a full clear and rescan only once. When I update tags I usually ensure that all of the timestamps in a particular album folder are updated to ensure that it is picked up correctly.
Also, if I'm changing any filenames to correct any typos I'll first remove the album folder from the library directory, scan for new and changed to remove the album, modify the filenames and put it back. Second new and changed scan and it's there correctly.
