Not Skype, but one of my clients takes a lot of feeds via Barix
Exstreamer, Tieline Bridge-IT, and Comrex BIRClink. Some of the Bridge-It
remotes are done using iPhones and Report-IT software.
The Bridge-IT is hooked to feed audio to two studios and also to an input
of a Broadcast Tools 16x4 switcher. One of th switcheroutputs is wired to
feed the Bridge-IT for monotoring purposes in the field; that way, it can
be used from either studio, or no studio at all.
